Agencies in good standing that complete accreditation are listed in the national SVI Directory and may display the SVI mark. Individual supervisors who complete credentialing may use the CSVP designation. Submit the form below and the Office of the Registrar will respond with the applicable requirements packet.

Submission of this form begins the intake process only. Accreditation and credentialing are conditioned on satisfaction of all requirements, including background adjudication.
